Form Has Long Accepted Medicare Coverage for Care and Is the Only Digital Health Provider Participating in all Direct-to-Consumer Pathways, Providing Eligible Beneficiaries with a Trusted Clinical Pathway to Access Obesity Treatment
Form Health, the national leader in science-based obesity and cardiometabolic care, today announced it has begun prescribing through the new Medicare GLP-1 Bridge Program, a new demonstration program that expands access to certain GLP-1 medications for eligible Medicare Part D beneficiaries with obesity. Having long accepted Medicare, Form is among the few digital healthcare providers with the clinical experience and infrastructure needed to help eligible beneficiaries access GLP-1 medications for weight management as part of comprehensive physician-led obesity care.
In addition to its longstanding Medicare participation, Form uniquely coordinates with key manufacturer programs, ensuring eligible patients receive comprehensive support through a single, physician-led obesity care provider.
“The Medicare GLP-1 Bridge Program represents an important step toward expanding access to evidence-based obesity treatment for millions of Americans," said Evan Richardson, founder and CEO of Form Health. “Because we've long cared for Medicare patients and are the only company to partner with both pharmaceutical leaders in this space, we're able to help beneficiaries understand their eligibility, navigate this new pathway, and receive comprehensive care that goes well beyond a prescription. Our goal is to make access as seamless as possible while delivering the clinical support patients need to achieve lasting health outcomes."
The Medicare GLP-1 Bridge Program is a new CMS demonstration running from July 1, 2026, through December 31, 2027, that expands access to certain GLP-1 medications for weight management for eligible Medicare Part D beneficiaries. The program operates outside the standard Medicare Part D benefit, requiring participating clinicians to determine patient eligibility, submit prior authorizations through a centralized processor, prescribe therapy when appropriate, and provide ongoing clinical management throughout treatment.
As demand for the new benefit grows, Form is available to help eligible Medicare beneficiaries access physician-led obesity care and prescription medications through this new pathway, ensuring patients have access to experienced clinicians as interest in the program expands. Rather than treating GLP-1 medications as a standalone solution, Form's American Board of Obesity Medicine-certified physicians and multidisciplinary Care Teams integrate medication, when appropriate, with nutrition guidance, physical activity, and behavioral support as part of comprehensive, evidence-based obesity care.
