While media often portrays DID personality states with distinct traits, such as different names, voice tones, or hair styles, individuals with the condition do not have to display these distinctions to be diagnosed. Medically, DID is characterized by two or more information processing centers in the mind that remain separate enough to have their own sense of personal identity, autobiographical memories, and the capacity to control behavior. These personality states may shift or overlap in ways that cause significant disruption to daily functioning and can result in a distorted perception of identity.

AMFM says that DID can be challenging to diagnose because individuals may lack awareness of their condition. However, one notable symptom is amnesia: patients often experience substantial gaps in memory and may forget about their life history, certain skills, important events, or things that they have created or purchased; they may also “get lost” in familiar places or be unable to recall reasons for travel. Other symptoms include out-of-body experiences, a sense of disconnection from the world, seeing and hearing things that others do not perceive, and feelings of division between separate selves.

Because DID often develops as a result of childhood trauma, trauma-informed care remains one of the most effective treatments for the condition. Specific therapies may include cognitive behavioral therapy and dialectical behavior therapy, two modalities that focus on helping individuals process unresolved trauma and develop coping skills; treatment for DID will also involve integrating separate identities to stabilize the patient’s sense of self and reduce the amount of disruptions they may experience in their daily life.

As separate identities often have their own needs, AMFM’s guide stresses the importance of working with a provider with experience in treating DID. Support systems outside of therapy are also very beneficial, as they can provide patients with a sense of community and allow them to connect with others who share their experiences.
